Here you will find the forms which must be completed to register with the Summit Area YMCA Seals. All forms may be uploaded and printed from this site, then returned to the Summit Area YMCA Aquatics Office.

For any family for whom the cost of the team and/or membership would cause a hardship, information regarding financial assistance from the Summit Area YMCA can be found at www.summitareaymca.org/area/financial.html.
USA Swimming Transfer Form This form must be filled out only by swimmers who swam for a team other than the Seals in 2008-2009 and returned with a $5.00 fee (checks made out to SSPO).
