Here you will find the forms which must be completed to register with the Summit Area YMCA. All forms may be downloaded and printed from this site, then returned to the Front Desk at Summit Area YMCA .
SAY Swim Team Costs and Payment Information 2011 - 2012 - Complete costs and payment information including payment plan information. Please read the ENTIRE document. Payment plan information is located on the 3rd page of the form.
NJ State Medical Form
2011 - 2012 Transfer form - This form must be filled out only by swimmers who swam for a team other than Summit in 2010-2011 and returned with a $5.00 fee (checks made out to NJ Swimming with Swim Team Transfer noted in the memo field).
